2012-2013 Financial Aid forms are available in both Spanish and English
Here's what you need to do:
1) Click on the following forms to download. All files are PDF documents.
2) Complete the required forms (See A & B below).
3) Enclose copies of all necessary tax returns and required documents.
4) Enclose a $24 check or money order made out to PSAS.
5) Send all of these by Fri., January 16, 2012 for priority consideration. Send forms/fee directly to:

There are two ways to fill out the form:
A) Print it out. Fill it out by hand.
B) If you are running Acrobat 7 or higher, the Student Aid Form PDF can be completed directly on your computer screen by placing the cursor at the appropriate fields and typing in the information. Print the completed document and save it on your computer for your records. NOTE: You can select which pages you would like to print under the "Print Range" section and preview every page under the "Preview: Composite" section by moving the sliding bar beneath the image.

When the application is completed, send the printed application, all required documentation and a check or money order (made out to PSAS) in the amount of $24 to the address listed above.
